I (Participant) am participating in classes/services/programs (YOGA with YOGI J classes, sessions, services, packages, events) during which I will receive information and instruction about yoga and health. I recognize that yoga requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any physical fitness program, including yoga. I represent and warrant that I have no medical condition that would prevent my participation in physical fitness activities.

In consideration of being permitted to participate in the yoga classes, I agree to assume full responsibility for any risks, injuries, illnesses(such as COVID-19) or damages, known and unknown, which I might incur as a result of participating in the classes. In further consideration of being permitted to participate in the yoga classes, I knowingly, voluntarily, and expressly waive any claim I may have against the instructor, the owner, or the leaseholder of the building for injuries, illnesses (such as COVID-19) or damages that I may sustain as a result of participating in classes.

While we (YOGA with YOGI J) try and ensure that all details, descriptions, and prices appear on our site(yogawithyogij.com), and this form is accurate, errors may occur. If we discover an error in the price of any services/packages which you (participant) have registered for, we will inform you of this as soon as possible and give you the option of reconfirming your registration at the correct price or canceling it. If we are unable to contact you, we will treat the registration as canceled. If you cancel due to an error on our part and you have already paid for the services, you will receive a full refund.

Payment for the package(s) must be made before the 1st session.

We may, at any time, and at our sole discretion, modify these Terms and Conditions, with or without notice to the participant(s) through email. Any such modification will be effective immediately upon public posting on our site. Your continued use of our services following any such modification constitutes your acceptance of these modified Terms and Conditions.
